Wide defects

Straumann® Emdogain PLUS is indicated for all intrabony defects in which tissue support and stability is required.

Treatment of wide defects

Straumann Emdogain PLUS - wide defect

Anesthetize the operative area selected for surgery by block and/or infiltration anesthesia. Avoid injecting a local anesthetic containing adrenaline in the interdental area or in the marginal gingival adjacent to the periodontal defect which is to be treated.

Remove any plaque, calculus and granulation tissue as necessary.

Remove the "smear layer" by conditioning the root surface with Straumann® PrefGel 4 for two minutes. Rinse thoroughly with saline solution. Avoid contaminating the cleaned and conditioned root surface with excessive blood or any saliva after the final rinse.

Immediately apply a thin layer of Straumann® Emdogain onto the exposed root surface, starting at the most apical bone level. Avoid accumulation of gel within the base of the defect, as this will compromise placement of the mixed formulation. In order to facilitate containment of Straumann® Emdogain PLUS, presuturing is recommended. Use sutures of a non-irritating material.

Straumann Emdogain PLUS - wide defect Add Straumann® Emdogain drop-wise to the Straumann® BoneCeramic particles in the inner blister until the mix gets a paste-like/wet coarse sand consistency. The remaining Straumann® Emdogain is then applied directly on the root surface (step 5).

Gently fill each defect with the mixed material, in small increments, to the highest level of the bone defect. Regeneration will occur best when blood and blood vessels can infiltrate the graft material. To allow for this blood vessel ingrowth, avoid compressing the material.

Primary closure and optimal wound stability should always be the goal. If it is judged necessary for good primary closure, the flap can be lengthened by cutting through the periosteum in the muco-gingival fold. To optimize wound healing, it is recommended to apply any remaining Straumann® Emdogain onto the sutures as well as along the wound margins.
